San Bernardino Urological Associates provides 24 hour emergency coverage for our patients. In case of emergency, our patients can call our office number and will be able to reach the urologist on call. We ask you to be considerate and utilize the call line for true emergencies.
Our office is open from 8:30 am to 5:30 pm on weekdays. All visits are by appointments only. We will try to see patients as soon as possible. Any patients who desire to be seen the same day because of an emergency will be seen that day. In case we cannot accommodate the patient, the Urologist will call the patient to decide the urgency of the medical condition. Friday afternoons are reserved for emergency appointments only.
We are Medicare Providers and will accept Medicare's fee schedule. We are contracted with most of the Managed Care groups in the Inland Empire. If you are a managed care patient, we ask you to be patient with the referral and authorization system. In general after your initial visit, our office will get the follow-up referrals, whether it is a follow-up visit, a radiographic examination or surgery. Please call us if you do not hear from us within 10-14 days. We do not take standard Medi-Cal patients, but work closely with IEHP and Molina. It is necessary for your primary doctor to call and scheduled the initial appoitnment with our office. Copays must be paid at the time of service and cannot be waived.
Our office adheres to the strictest criteria when it comes to physician/patient confidentiality. Your medical records and medical conditions will not be released to anyone without your written authorization. Please allow us 7-10 days to copy your records when you request them. Record fee is based on the quantity of pages prepared.
Disability forms can not be completed before the surgery date or after disability begins. There is a fee for completing the form(mulitple forms will require additional payment) and that is expected to be paid before the forms are completed. Most EDD(state disabiilty) forms can be completed electronically once the patient has registered and opened a case at http://www.edd.cagov/disability . FMLA and all other forms will be completed once fees have been paid. Please allow 3-5 days for forms to be completed.
